1.0 PURPOSE

To ensure protection and containment of the Personal Health Information (PHI) and disability documentation of the students served.

2.0 POLICY

2.1 Policy Statement

Records of personal health information relevant to the disability documentation process will occur within the student disability accommodation record.

2.2 Definitions

  • PHI - Personal Health Information
2.3 Procedure
The Disability Services Director and staff will ensure safe, secure and confidential management of disability records.

3.0 AUTHORIZATION  

The Director of Disability Services has the authority to change, modify, or approve exceptions to this policy at any time, with or without notice, in accordance with federal disability regulations or modifications and amendments to those regulations, with the approval of the Chief Academic Officer and the President of the University or his designee (Compliance Coordinator).
