The Welfare Benefit Plan - Summary Plan Description (hereafter referred to as the "Wrap Document") is a legal document used to bring all health and welfare plans under one "umbrella mega-plan" and into compliance with ERISA requirements.


2.1 Policy Statement
The Welfare Benefit Plan - Summary Plan Description must be approved by the University Board of Trustees (original approval date: December 12, 2014), reference current health & welfare summary plans, and be distributed to all employees. The Plan must be made available to all employees as a reference upon demand. Subsequent modifications are governed by this policy and federal regulation.

2.2 Definitions

2.2.1 Welfare Benefit Plan - Summary Plan Description - A document that "wraps" around the insurance policies, coverage certificates, and plan booklets, supplementing them with the information necessary to comply with ERISA. The benefits available under the plan continue to be governed by the prevailing insurance policy, coverage certificate, or plan booklet where available.

2.2.2 Wrap Document - Colloquial term used herein to reference the Welfare Benefit Plan - Summary Plan Description.

2.3 Procedure

2.3.1 The Wrap Document is a legal document prepared by ERISA attorneys on behalf of the University; it becomes official upon approval by the University Board of Trustees.

2.3.2 The plan must be distributed to all current and former employees who utilize the University's health & welfare plans. Distribution can be electronic provided strict rules and procedures governing distribution are adhered to and hard copies are provided to those who don't have electronic access or who request hard copies in lieu of electronic copies. Hard copies can be provided to any employee upon request.

2.3.3 Distribution will be made by the Human Resource Department as required by federal regulation. The document will also be posted on the University Human Resource Department website.


The Director of Human Resources, in conjunction with the VP for Administration, has the authority to change or modify this policy at any time provided such changes are in compliance with legal or regulatory requirements, University compliance program requirements, and other policy guidelines. Such changes to the attached Welfare Benefit Plan - Summary Plan Description must be approved by the President or his designee after communication with the Board of Trustees (BOT). Material changes may need to be pre-approved by the BOT.
