INSTRUCTIONS/CHECKLIST FOR APPLICATION

We begin to look at applications for the Fall (September) classes in January of the same year of expected enrollment. It is in your best interest to apply early in the admission cycle for which you are enrolling. You will be contacted for an interview and/or campus visit. Keep this form for your records.

All materials should be sent to the above address. The Application form should be submitted as well as the following items:

Three letters of recommendation.   The letters should have contact information and the position of the person providing the recommendation. At least one recommendation should be from a person who can testify to your ability to work with people.

Official Transcripts from all postsecondary institutions attended.

Essay:  Comment about your unique life experiences and community service, which have helped you develop strong morals & ethics and has given you leadership skills. Indicate how this has prepared you to interact effectively with different types of people.

Scores for the Graduate Record Examination (GRE) should be mailed directly to the Department of Physical Therapy from the Educational Testing Service.  Our institution code is 5906; the department code is 0619.  See their website: www.gre.org.

Application fee of $25.00; make checks payable to Wheeling Jesuit University.

A resume in the format outlined in, “Guidelines to Preparing the Resume.”

Eighty volunteer/observation hours (Please submit a letter from the physical therapist/supervisor(s) confirming the number of volunteer/observation hours that have been completed at a clinic or hospital).
