ALL STUDENTS MUST HAVE MEDICAL INSURANCE COVERAGE. 

Students must provide a xerox copy of the front and back of their current insurance card as verification of coverage.  Medical insurance information will be kept confidential and on file for scheduling medical referrals to outside physicians, outpatient treatment, emergencies, and/or participation in University athletics.
