Immunization requirements: An accurate record of immunizations is required as part of the admissions procedure for all students born after 1956. This may be obtained from your private physician, health department, or high school.

It is mandatory that you have:

• Tetanus-Diptheria-Pertussis vaccination or booster within the last 10 years is required. A substitute one-time dose of Tdap for Td booster; then bosst with a Td every 10 years for persons aged 19-64.
• Polio vaccination series
• 2 MMR vaccines – (Measles, Mumps, and Rubella)
• Hepatitis B vaccines (a series of 3 injections) OR a titer (labwork) showing immunity OR complete and sign the immunization waiver/disclaimer form. Please note: health science students cannot waive the Hepatitis B series.
• PPD (Tuberculosis test) within the last year, prior to admission.
• Note: All international students are required to have PPD testing upon admission regardless of previous BCG vaccination.
• Meningococcal (Bacterial Meningitis) OR complete and sign the immunization waiver/disclaimer form.
• Varicella Vaccine, recommended by the American College Health Administration (ACHA)--if you have not had chickenpox as a child.

Physical Examination: A current physical examination (by a physician or nurse practitioner) is required for all incoming students. Please note that yearly physicals are required for athletic participation and that an athletic physical meets our pre-admission requirements. In addition, the Centers for Disease Control recommends that a gynecological exam and Pap test be done for females age 18 and older (please include this in your pre-entrance examination). If you wear glasses or contact lenses, we recommend that a copy of the prescription be sent with the health form to spare unnecessary delays, if urgent replacement of glasses or contact lenses are needed due to breakage or loss.

Other: If you are having or have had treatment for an emotional disorder, an addiction to alcohol or other drugs, or a serious medical problem that you wish to report in more detail, please include this in a separate envelope addressed to the university counselor and/or nurse and mark it CONFIDENTIAL.

The pre-entrance medical form, including physical and immunization records, must be included and returned to Wheeling Jesuit University before your admission is final. You will not be able to register for classes if your records are incomplete. Thank you for your attention to this detail and welcome to our school.
